The Rochester Public School's Licensed School Nurses
will be starting district-wide vision and hearing screenings. Screenings at Folwell Elementary will be done

Hearing screenings will be done on:

not have a screening on file with Rochester Public Schools or who have
failed their previous screening.
- Kindergarten students will be
screened, if they failed their preschool screening.
- Students may also self-refer
or be referred by the parent/guardian, or by staff members to rule out a
hearing or vision concern.
Vision

graders and those students in grades 2nd, 4th and 5th who do not have
a screening on file with Rochester Public Schools or who have failed their
previous screening.

Vision and hearing are critical to learning and the RPS
health staff strongly encourages that students be screened.
If you wish to decline either of the
screenings, please contact Nurse Kelly at keleadens@rochester.k12.mn.us
or call your child’s building by Friday, January 19, 2018. Please indicate your child’s name, grade and
teacher, along with which screening you do not want us to do.
If your child is screened, you will only be contacted, if
we believe your child may need a professional evaluation.
